By accepting these terms when booking this facility, you acknowledge that you have read and understood the document in full, and agree to all terms stated therein.

I acknowledge and understand that there is an inherent risk of injury associated with the use of indoor cricket nets and equipment, including cricket balls and bowling machines. I recognize that participation in these activities involves potential hazards, which may include but are not limited to:

(i) sprains;
(ii) strains;
(iii) fractures;
(iv) facial injuries;
(v) dislocations; and
(vi) heart attacks.

I appreciate these risks and accept full responsibility for my participation.

I further acknowledge that B3 Cricket recommends the use of the following protective equipment to reduce the risk of injury:

• Abdominal guard (cricket box)
• Batting pads and thigh guards
• Batting helmet
• Batting gloves

I understand that these activities are physically demanding and require a good level of fitness. I accept the associated risks and confirm that I am physically able to take part.

I acknowledge and agree that I am responsible for my own safety, the safety of my possessions, and the safety of others when participating in the activities. I understand that an adult supervisor will be present to oversee activities and that no person under the age of 18 is permitted to operate the bowling machine.

In the unlikely event of an accident, or loss or damage to personal belongings, I acknowledge that B3 Cricket will not be held liable for any direct or indirect loss, damage, or injury arising from or in connection with the activities (except in the case of death or personal injury resulting from B3 Cricket’s negligence.) I hereby waive all and any claims against B3 Cricket in this regard.

I confirm that I, along with any members of my group, have been provided with the Cricket Nets Safety Briefing & Guidance Document, which is also available on the B3 Cricket website and displayed within the indoor nets facility.

Known Medical Conditions

For your safety and the safety of others, if you answer YES to any of the questions listed below, you will not be permitted to use the indoor cricket nets or participate in any related activities at this facility.

Please answer the following:

1. Have you been advised by your GP or a medical professional that you have a heart condition?

2. Have you been warned not to participate in physical activities by a healthcare provider?

3. Do you experience chest pain during physical activity?

4. Do you suffer from dizziness, or have you ever lost consciousness?

5. Do you have any bone or joint problems that could be aggravated by physical activity?

6. Are you currently taking medication (e.g., water pills) prescribed for high blood pressure or heart conditions?

7. Is there any other medical reason you are aware of that would prevent you from safely taking part in physical activity?

By accepting these terms and conditions when booking this facility, you confirm that all information provided is accurate and complete to the best of your knowledge.

You confirm that you have answered “NO” to all of the above conditions and are happy to proceed with participating in the activities.

Please be aware that it is the responsibility of the activity participant, or the guardian of a minor (a minor being a person under the age of 18), to answer these questions as accurately and truthfully as possible, and to the best of their knowledge.

Upon completion, B3 Cricket will use the information provided to assess the participant's risk level for engaging in its activities.
